How should I journal symptoms for my EP appointment?

Short answer

Use a one‑row‑per‑episode log with: date/time, HR/BP (if you have it), a concise symptom description + duration, context/triggers, meds/dose, and one question you want to ask. Before the visit, create a one‑page summary of your top three episodes with what helped/worsened them.

What to record (and why)

Field Why it matters Example
Date & time Correlate with device logs and daily rhythms. 2025‑10‑27, 22:40
Heart rate / BP Context for autonomic tone and exertion. HR 66 bpm, BP 118/72
Symptoms + duration Primary signal for urgency and patterning. “Short tapping near sternum for ~20 sec.”
Context / triggers Links physiology to perception. After rowing; slightly dehydrated; left‑side lying.
Meds / dose Detects timing interactions and side effects. Metoprolol 25 mg at 20:00
One question Focuses the visit and shared decisions. “Could rate‑response settings explain the tapping?”

Prefer concise, factual notes. Avoid personal identifiers if you plan to share.

When to seek urgent care

Call emergency services for new chest pain, fainting, stroke symptoms, severe shortness of breath, or a new constant device sensation. Do not wait for a routine appointment.
