Headline items

  • High‑sensitivity troponin T = 49 ng/L elevated → indicates myocardial injury. Repeat hs‑TnT in 1–3 hours and correlate with EKG and symptoms to distinguish heart attack from other causes (arrhythmia, demand ischemia, myocarditis, PE, etc.).
  • NT‑proBNP = 328 pg/mL just above “rule‑out” threshold; far below typical rule‑in levels. Interpret with symptoms/exam.

Practical plan to discuss with your care team

  1. Repeat hs‑troponin in 1–3 hours; compare with prior values, review EKG and symptoms (chest pressure, dyspnea, syncope). Elevation + rise/fall + ischemic evidence ⇒ treat as MI; otherwise evaluate non‑ischemic causes (including brady/tachy episodes or nocturnal non‑capture).
  2. NT‑proBNP: treat 328 pg/mL as indeterminate; correlate with exam, chest imaging, and ultrasound if dyspneic.
  3. Anemia workup: reticulocyte count; iron panel (ferritin, iron, TIBC), B12/folate, TSH; check stool/urine blood as indicated and trends on prior CBCs.
  4. Platelets 146: recheck on fresh sample; review meds/supplements, alcohol, and recent infections. Escalate if trend declines or <100.
  5. Basophils: if allergy symptoms present, likely reactive; otherwise trend. Persistent >0.4 ×10⁹/L → hematology input.
  6. CO₂ 21: if symptomatic, repeat chemistry ± blood gas; otherwise trend.
  7. Glucose: clarify if fasting; obtain A1c.
  8. Kidney function: add urine albumin/creatinine ratio to complete KDIGO staging if concerned.
Urgent symptoms? If chest discomfort, new/worsening shortness of breath, fainting, or concerning symptoms occur, seek immediate care.
